A New Free Research Tool to Assess Transition into a Nurse Leader Role

Nurse leaders are always looking for ways to improve the development and transition of their new leaders. A good place to start is by asking your current leaders about their own leadership transitions.

Many of you may be familiar with or already using her widely used new graduate transition tool. This leader transition instrument (tested for reliability and validity) is now available for your organization’s use or for your DNP or PhD research.

From Bedside to Leadership: Understanding the Challenges of Transitioning into a Nurse Leader Role

Transitioning from staff nurse to nurse leader marks a significant professional milestone. This change brings not only excitement and new opportunities but also increased responsibility and the potential to influence nursing practice at a broader level. However, stepping into a formal leadership position introduces a complex set of expectations, responsibilities, and personal adjustments. Without intentional support and adequate training, these challenges can quickly become overwhelming.

Challenges Faced by New Nurse Leaders

New nurse leaders often encounter difficulties as they adjust to their expanded roles. Navigating difficult performance conversations can be stressful and uncomfortable. Another challenge is developing a solid understanding of the business side of running a unit or department, such as managing budgets, interpreting financial reports, and aligning departmental goals with broader organizational objectives. These business acumen skills are frequently new to clinicians and require focused training to build knowledge and support. These experiences underscore the importance of recognizing and measuring the specific challenges associated with transitioning into nurse leadership roles.

The Importance of Supporting Emerging Leaders

As healthcare systems continue to address workforce challenges, ensuring a smooth transition for emerging nurse leaders has become increasingly important. Gaining a clear understanding of the difficulties new leaders face and having reliable ways to measure their experiences provides organizations with the data needed to develop stronger, more sustainable leadership development programs.

Introducing the Casey-Fink Nurse Leader Role Transition Survey ©2025 

To help organizations better understand and support novice nurse leaders, the Casey-Fink Nurse Leader Role Transition Survey was developed. This new, evidence-based instrument is designed specifically to measure the experiences, confidence, and support needs of nurses stepping into their first leadership roles. The survey helps organizations identify gaps in the transition process, enhance onboarding, and provide targeted support for emerging nurse leaders.

An expert panel, which included nurse leaders and researchers, reviewed each survey item for relevance and clarity. Psychometric properties were analyzed with responses from 477 nurse leaders with 24 months or less experience in the role. The analysis demonstrated strong reliability and validity across all ten subscales, offering a comprehensive picture of nurse leader responsibilities and providing empirical evidence to inform improvements in onboarding programs. The subscales are titled:

  • Business Acumen
  • Adaptability
  • Quality Outcomes
  • Advocacy

  • Influence

The Impact of Strong Nurse Leadership

Strong, well-supported nurse leaders are essential for fostering healthy work environments, improving patient outcomes, and developing engaged, resilient nursing teams. By capturing the real-time experiences of new leaders, organizations gain valuable data that can inform leadership development strategies, onboarding plans, curriculum design, mentorship programs, and resource allocation. Investing in the transition experiences of new nurse leaders is critical to organizational success.
